Performance Analysis Highlights 2009

 

The Port Alberni Association for Community Living (PAACL) has produced a Performance Analysis Report annually since 2004.  The information is used to improve our programs and services.  The Performance Analysis Report shows outcome results and program satisfaction. 

 Below are the highlighted areas for performance improvement and the status of the previous year’s identified areas of improvement.  A full copy of the Performance Analysis Report can be found on the PAACL website; www.paacl.ca

Day Program, Employment and Leisure Services

 Semi-Independent Living & Self Help Skills Programs (Services for Community Living)

 The program served 52 individuals in 2009.  The program is operating close to capacity. 

 Accomplishments in 2009

·         Adult Services staff received PATH training the beginning of 2009. Staff are now facilitating PATH meetings with participants annually.

·         Services for Community Living staff, with support from other program staff have developed a list of individuals wishing to live more independently. Information is obtained through PATH meetings and support sessions with participants.

·         Services for Community Living staff now use the PATH format to identify and track goals. Throughout Adult Services, many services and activities are offered to all PAACL participants, providing an efficient way to assist participants in meeting their goals.

 What We Are Working On In 2010

·         Continue PATH format for planning participant program plans.

·         Maintain a list of individuals interested in obtaining (semi)independent housing.

·         Ongoing assessment of participant goals to determine the most effective way to assist them in meeting their needs.

Access Leisure Support Program

 The Access Program provided support for 33 individuals in 2009.

 Accomplishments in 2009

·         A group of four individuals were given training in peer mentoring and provided assistance in organizing their own unsupervised activities. These four individuals are quite independent, and despite some initial setbacks, continue to access community leisure activities.

·         The Community Access staff continues to seek out new recreational opportunities to suggest to program participants.

 What We Are Working On In 2010

·         Offer peer mentoring training to interested Community Access participants.  The goal is to assist participants in meeting their recreation goals more independently.

·         Provide support in community based activities that meet the budget requirements of participants and the program.

 Community Employment Program

 C.E.P. served 18 individuals in 2009.

 Accomplishments in 2009

·         CEP organized the Port Alberni Employment Networking group. The purpose of this group was to learn about employment services located in the community in order to facilitate the most effective referrals for participants.

·         An application was made for a Youth Transition program which would provide employment/life skills to youth. PAACL is waiting for a response to this application.

 What We Are Working On In 2010

·         Continue community education about CEP and other PAACL programs through the monthly news articles in the AV Times.

·         Continue to apply for new funding via grant monies for employment projects.

 Connections Day Program

 34 participants were supported in 2009 at Connections. 

  Accomplishments in 2009

·         The PATH system is used throughout Adult Services for participants and to track program goals.

·         The Connections team has regular bi-weekly staff meetings. The team has had two meetings where interpersonal conflicts were addressed.

·         PAACL is in the process of securing funding for the Snoezelen Room project.

 What We Are Working On In 2010

·         Work toward a more effective and cohesive team by continuing with quarterly meetings and team development days.

·         Continue raising funds for the Snoezelen Room project.

·         Maximize opportunities for participants to meet their goals in an efficient, cost-conscious manner.

  Pathways

 Combined attendance for Pathways for 2009 was 3219. This is an increase of 672 drop-ins from the previous year.

 Accomplishments in 2009

·         20 of the 22 participants (91%) that responded to the satisfaction survey indicated that they are involved in the decisions made in the program.

·         Renovations were completed to open up the space used for the program.  A second staff was hired to meet the needs of the increasing number of participants.

 What We Are Working On In 2010

·         Continue to encourage people to be more independent and take ownership of their clubhouse.

·         Foster development of natural supports.

·         Involve community members in Pathways promotes community inclusion.

 Residential Services

 During 2009 twenty one individuals were supported in our residential programs, some of the individuals were supported for respite and others have made it their home.

 Accomplishments in 2009

·        A Death Policy was developed to refer to during these difficult times and the Medication Procedure was revised to include decision making authority of the individuals in care and also include the necessary information for individuals responsible for their own medication administration and decision making.

·         There is an intake package in place, though a more comprehensive intake package would be useful. This item will be forwarded to be completed in 2010.  

·         Individuals that reside in a PAACL residences drop in to other PAACL programs to take part in activities or to visit with their peers. There is also an effort made to plan trips with other residents with similar interests as a way to build social networks and to share costs and resources.

 What We Are Working On In Residential Programs 2010

·         Have a member of the residential management team develop a new comprehensive intake package to be used for new residents when they require respite or long term support in a residential program. 

·         Work on outcomes measures that are reflective of the goals of the residents in care. 

·         Improve communication between programs with respect to activities and community events so that residents can pool resources and share successes.

 Host Families

 PAACL has one host family model.

 Accomplishments in 2009

·         A residential home is now provided for the day program as well as respite.  This home is available in the event of an emergency that may cause a gap in residential service.  It has also been beneficial in the recruitment of respite staff that would not be able to provide this service out of their own home.   

·         A portion of 24 hour casual respite services is now counted toward staff seniority.  This incentive has proven beneficial in obtaining and maintaining respite staff.

What We Are Working On In 2010

·         Continue toward the development of an emergency plan which would include the provision of staff in the event that the caregiver is unable to provide support.

·         Have a satisfaction survey completed by a family member of the individual in care.

 Children’s Services

 Infant Development Program

 In 2009, the program served 87 infants and their families located in Port Alberni, Ucluelet and Tofino. 10 children were supported in Ucluelet, 4 in Tofino and 73 in Port Alberni. The program received 52 new referrals. 

 Accomplishments in 2009

bulletIDP staff were able to make contact with families/caregivers within 7 days of a referral, this is an improvement from last year where it typically took 17 days.  Children referred to the program had service within 30 days.

·         100% of children on the caseload made progress towards their identified goals.

 What We Are Working On In 2010

·         Increase the number of community partners/resources to offer more services for families in a group setting.

·         Focus on working with the community to decrease the number of children in care.

·         Maintain the average time from referral to first contact.

 

Supported Child Development

 The program provided support to 64 children. The program supported 2 children in Ucluelet, and 62 children in Port Alberni

 Accomplishments in 2009

·         The demographics collection has increased and helped us to see what identify a change in the needs of the children we are seeing referred to the program which has assisted with better planning.  (i.e. Diagnosis)

·         Although SCD consultants try to contact referrals within 14 days reaching 100% of children’s guardians is difficult because calls are not always returned. 

·         95% of children accepted to the waitlist are receiving consultation support within 90 days.

·         SCD staff continue to provide program information to the community through pamphlets and consultants and by attending advisory and other community meetings.

·         Some children are able to receive shared support.

Recommendations for 2010:

·         Expand demographics to reflect Ethnicity.

·         Maintain the average time from referral to first contact.
